Craig Boswell, a 53-year-old engineer from Stirling trading as Amusement Inspection Services, issued the M&D Theme Park with a declaration of operational compliance for its Tsunami ride on 10th June 2016. On 26th June five cars derailed at 40mph, injuring 10 people.
The Health & Safety Executive (HSE) is still investigating the cause of the crash but has served the ride inspector with a prohibition notice, which prevents him from undertaking any inspection of fairground rides until the condition of the prohibition notice is fulfilled.
Photographs after the incident show that wheels came off the cars, but it is unclear whether this was the cause or a result of the derailment.
The HSE said that was working with industry bodies to ensure all fairground owners carried out new independent checks on rides previously cleared by Mr Boswell.
